Please sign and return the following forms that will be sent home by no later than Friday, September 11th.
▪ Signed “Student Data Verification Form” (updated as necessary)
▪ Signed and completed “IS-09-L-E Notification and Permission” form.

Student Accident Insurance: Parents choosing to purchase student accident insurance are encouraged to use the on-line link http://www.insuremykids.com .
Some reminders:
- Required forms must be returned by Friday, September 11th so that we can finish our organization properly. Please remember to sign them.
- Student photos will be taken on Monday, September 14th.
- School Council Meeting – Monday, September 21st at 6:30 p.m. in our Library.

The following items will be available for purchase from the school. Students/families requiring financial assistance should contact any staff member. The staff member will ensure that the student’s issue is dealt with in an appropriate, sensitive and confidential manner.
- Abus Lock – $5.00 (encouraged to purchase from the school because of the required etched serial number on the bottom/back)
- Westheights School Student Planner – $5.00
The following is a list of supplies that students were required to purchase.
- pencils, pencil crayons, erasers, highlighters
- glue stick, 3 ringed lined and blank paper
- ruler
- USB stick
- four 1.5 inch binders (different colours)
- pencil case (fabric case preferred)
Personal devices (smart phones, ipods, ipads, chrome books, etc.) are welcome.
